Sign in to Google
Click on More on the black task bar, and select Documents.
Click the red Create button on the side, and select Form.
Title your form where it says “Untitled form”

Sign in to Google
Click on More on the black task bar, and select Documents.
Click the red Create button on the side, and select Form.
Title your form where it says “Untitled form”
Type in your first question in the “Question Title” box, and add help text if
needed.
Select your Question Type from the drop down.
Check the box if you want to make the question required and click the done
button.
Repeat the process for question 2.
Click Add item at the tope to add more questions, pick your question type.
When you are finished go to the Theme button at the top and choose a theme, if
you like it, choose Apply.
At the top of the form choose “require Parkhill.

collect respondent’s Parkhill.

Click the link at the bottom of the page to go to your live form.
Copy the link to
share with others OR select Email this form to share with others.
