The aetiology was the presence of ectopic third molar in the sinus. Closure of the communication and surgical removal of the tooth via Caldwell-Luc procedure resulted in resolution of the symptoms. The oroantral communication was surprisingly caused by the chronic sinusitis resulting from the presence of the tooth in the maxillary sinus.
